Dear 5th Grade Students and Parents,
Our 5th graders will soon be leaving Benfer and moving onto Strack. As a gift, the 5th Grade Appreciation Committee would like to provide each student with a memory book filled with photos, quotes, and memorable moments from their 5th grade year. You are invited to purchase a dedication to your child which will be printed in this memory book. Please note that we are NOT limiting dedications to just parents, so each child may receive multiple dedications. Grandparents, siblings, aunts and uncles, neighbors, teachers, coaches, scout leaders, and anyone else are all invited to submit a dedication.
Prices are as follows:
Business card sized $15 – 1.5” x 3.5” fits 1 photo and 1 short message
1/4 page $25 – 3.5” x 4.5” fits 2 large photos and 1 short message
1/2 page $40 – 5” x 7” fits 1 large phot or up to 3 small photos with a short message
Full page $75 – please provide a full page ad already designed in a .jpeg format

If you would like to submit cash or check (made payable to BENFER ELEMENTARY) please send in a sealed envelope marked with 5th Garde Memory Book Dedication to your child‘s teacher (or the Benfer front desk) AND the bottom portion of this form.

You will get an email reply that your dedication was received. If you do not receive this reply then your dedication has not been received and should be resubmitted.
